![]() If we are not the primary care provider on your child's card, we may not be able to complete referrals, etc. You should receive a new card with the updated information. If your child has insurance through Medicaid, we ask that you update your insurance to include our office or one of our providers as your child's primary care provider. We do accept South Carolina and North Carolina Medicaid plans. Please check with your insurance prior to your first appointment to ensure that our providers are in-network with your insurance plan. If we are not a provider for your insurance or if you are not insured you may be responsible for the entire charge at the time of service. If you have a high deductible plan and have not met your deductible, please be prepared to pay for the office visit at the time of service. If we file your claim, you are responsible for the co-pay at the time of visit. If you do not have a copy of your child's insurance card, we may consider you selfpay until this information is provided. Please have your card with you at the time of your visit. We accept most major Private insurance carriers. Any other persons that may accompany your child to appointments must be first authorized in writing by the legal parent or guardian and must present an ID at check-in. The legal Parent or Guardian (with proper legal paperwork) must accompany the patient for the first appointment. You must bring your child's insurance card and your photo ID. ![]() ![]() To help expedite the first appointment, we ask that you please arrive 10 minutes early and bring your child's immunization records and be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.
